How do I request a Customer Number from the USPTO?

To request a Customer Number from the USPTO, follow these steps:

  1. Complete the Customer Number Request Form (PTO/SB/125)
  2. Include the names of registered practitioners associated with the Customer Number

  4. Submit the form to the USPTO
