What information should be included in the transmittal letter for a computer program listing appendix?

When submitting a computer program listing appendix, the transmittal letter should include the following information:
- A statement that the submission is a Computer Program Listing Appendix.
- The total number of compact discs (if applicable).
- The machine format and operating system used to create the compact disc.
- A statement that the computer program listing appendix is submitted on compact disc in duplicate (if applicable).
- A statement incorporating by reference the material on the compact disc into the specification.
MPEP 608.05(a) states: “The transmittal letter must include, in addition to any other needed information, the following information: (1) A statement that the submission is a Computer Program Listing Appendix.”
