What information is included in Form DM/1 for an international design application?

Form DM/1 for an international design application includes specific spaces for a wide range of information. According to MPEP 2909.01, the form includes spaces for:

  • Applicant’s entitlement to file an international design application
  • Applicant’s contracting party
  • Correspondence address for multiple applicants
  • Appointment of a representative
  • Number of industrial designs and reproductions
  • Designated Contracting Parties
  • Applicable product and Locarno classification
  • Description and description of reproductions
  • Identity of the creator
  • Claim and priority claim
  • International exhibition information
  • Exception to lack of novelty
  • Relation with a principle design
  • Publication request (immediate or deferred)
  • Reduction of individual designation fee
  • Signature of the applicant or representative

The form also includes a “Payment of Fees” section for identifying the payment method and instructing the International Bureau on fee deduction from a current account.
